Indications d'utilisation

Used for estimating kidney function in adults with suspected or confirmed CKD when serum creatinine and urine creatinine are available, particularly to pair with urine albumin for CKD detection and staging per KDIGO 1 [Level I evidence]. Clinically useful for initial CKD evaluation, risk stratification, and drug‑dosing decisions, but less accurate at high GFR and when creatinine generation is abnormal; confirm with eGFRcr‑cys or measured GFR when results will change management 2 [Level I evidence].

Informations et précautions

Produces estimates heavily affected by urine collection error—24‑hour CrCl shows ~25% mean bias with both under‑ and overcollection common 1 [Level I evidence].
Non‑GFR determinants of creatinine (muscle mass, diet, tubular secretion) further distort results.
Use only when mGFR is unavailable and eGFRcr‑cys is unreliable, ideally supervised to reduce error (KDIGO 2024).
