LDL‑C goals are used to guide risk‑based statin initiation and treatment intensity in primary and secondary prevention. 1 [Level I evidence]
- Indications: Adults 40–75 with LDL‑C ≥70 mg/dL and ≥7.5% 10‑yr ASCVD risk, diabetes, LDL‑C ≥190 mg/dL, or established ASCVD.
- Clinical utility: Target ≥30–50% LDL‑C reduction in primary prevention and ≥50% with LDL‑C <70 mg/dL in ASCVD; intensify therapy if above thresholds. 2 [Level I evidence]
통찰 및 주의사항
ESC 2024 recommends LDL‑C <55 mg/dL for very‑high risk and <70 mg/dL for high risk patients. 1 [Level I evidence]
Targets should be as low as possible; very low LDL‑C remains safe, supported by FOURIER‑OLE showing no excess adverse events at <20 mg/dL 2 [Level I evidence].
Caution: aggressive lowering requires monitoring for new-onset diabetes, muscle symptoms, and polypharmacy interactions.
